The standard format is 6 columns of information. Direct your QA Manager to submit your information in the standard format in the following order:

Date Submitted, Part Number, Manufacturer, Date Code, Lot Code, and Findings/Comments.

The Findings/Comments column will typically be limited to the following explainations:
